scsi-disk: restruct emulation: GET_CONFIGURATION

Move GET_CONFIGURATION emulation from scsi_send_command() to
scsi_disk_emulate_command().

Also add GET_CONFIGURATION to scsi-defs.h and scsi_command_name().
